If you configure the polycom with the correct digitmap, then you will
never have to hit send... :)

Because you haven't forwarded the call until after you hit transfer the
second time. If you hangup, the call should be on hold on your handset
still (ie, cancel the transfer)

At least doing it the wrong way doesn't hangup on the caller :)
