I tried this after reading one of your previous posts.  No matter
whether I use rfc2833 or inband, I'm unable to interact with remote IVR
systems (ie: my bank).  Using a Budgetone, Handytone, or Sipura, I have
no problem.  It's really quite odd, and I'm not too sure what I'm
missing.

The disconnect issue also still exists (for me) with 4.55 firmware.  I
can use the uniden to call another local sip phone (with
canreinvite=no), and leave both phones off the hook for as long as I
like.  However, if I use the Uniden to call an PSTN number (via tdm400p
fxo), then the uniden will start playing a congestion tone (not from
asterisk), and cease tx/rx audio.  After this happens, a 'show channels'
will show that the call is still active.

... and as I write this I wonder if I have discovered something that may
be the causing this problem.  I'm not an expert on the way sip
communications take place, but I do notice that ALL outgoing calls on
both my servers, never report the channel state accurately:

That Zap/3-1 should have a state of UP, not Dialing.  I noticed this
before, and just ignored it ... but perhaps it is the root of the
problem? Does * send special sip headers when a channel's state is UP? 
Perhaps the Uniden has a internal timeout, after which it will play a
congestion tone?
