Steve's posted a temporary solution under the thread I started (search for 
TEMPORARY FIX RC1 in the archives) -- basically disable jitter buffer on the 
machine you're converting from TDM to IAX.  In my setup:

home* - IAX2 - colo* - IAX2 - nufone

I disabled the jitter buffer on home* (and office*, which also connects to 
colo*).

Please refer to the other thread for more details, but this temporary 
workaround cured the gappy audio on my system.
