And few more:
 1. the global variables defined in extensions.conf are not recognized on
startup, but only after extensions reload
 2. chan_misdn is not created, because of failed dependency, while it is
actually met
 3. chan_h323 same as chan_misdn

 going back to r40700 fixes 1. and 2. (well chan_misdn is broken at this
point, but the dependency is met)
 3. is from a long time ago, but i do have openh323 instaled, but there is no
option in the makefile to provide its location
