[Asterisk-Users] Phone keypad input not working during "menu's"

I have a few Polycom 500's hooked up to my Asterisk box.

I have set up some extensions and the phones can call each other, 
conference call, transfer calls, hold calls, etc etc.  Everything seems 
to be working out great... except!  That when I have a menu setup or an 
interactive feature such as voicemail, the menu does not appear to 
receive the instructions from the phone.

For example, I dial the company directory (*411).  I get the digital 
assistant asking me to dial the first 3 number of the person's name.  I 
do that.  I do that and I push '#' afterwards.  I pound on the keypad 
like a crazed monkey.  I do nothing.  In all cases the same thing 
happens.   After a suitable pause, the digital assistant repeats the 
instructions.  After doing this a few times it hangs up.

Similar behaviour happens on any menu.

So it appears that Asterisk is not getting the keypad pushes I make once 
I've entered a menu.

If anyone has any ideas about what could be wrong, please let me know!
