[asterisk-r2] caso extraño MFCR/R2 Venezuela

Lo he utilizado con Elastix 1.6 (Asterisk 1.4.42 creo) y 2.0 (Asterisk 1.6
Branch Moy). Claro, recompilando todo lo relativo a Dahdi, asterisk
(haciendole el patch para dtmf-r2), y wanpipe (uso tarjetas Sangoma).
