I have tried various recommended providers and none have lived up to 
even a basic level of customer support expectations.

24x7 monitoring of support channels <-- note plural
responses to customer requests/tickets in a timely fashion
toll-free origination into asterisk via IAX
IAX to PSTN termination
web-based management panel (request new dids, billing info/balance, 
news/features/etc)
prepayment with reasonable MRCs per requested DID, no "account charges"

Optional, but highly sought: local did origination for North Carolina 
ratecenter: ROCKINGHAM

Optional, but interested: reseller/agent channel for easily reselling 
your services
