If you want asterisk to wait and see before answering, use the WaitForRing dialplan app. I used to use this when I had a fax machine that answered a specific dring and I needed to wait to see if that happened. If the line is answered within the timeout, the call is considered dropped before answering.

(You might ask why I didn't use dring detection - it doesn't work if you are also working with caller ID in the UK.)

As for detecting it after answer, I am not sure it's possible. Nor, sadly, is it possible to determine that the line is in use, as far as I know, if the use does not go through asterisk.
