[asterisk-users] o extension for voicemail app

I'm trying to set up the o extension for the voicemail app that will bring
the caller out if they go to someone's voicemail.  I've set the operator=yes
flag in the voicemail.conf file, I then added an o extension into
extensions.conf to point to a macro that I have created.  When I try testing
this by dialing a phone, waiting for it to go to voicemail, then pressing 0,
it plays the vm-sorry.gsm file and hangs up.  Is there something else I have
to modify to get this to work, I've tried entering the o extension into
other context as well and have tried to create a 0 extension with no luck,
it just plays the message "I'm sorry I did not understand your response."
