I think the problem is:

exten => 2,3,Set($ { DB( forward/${CALLERIDNUM} ) = ${FORWARD} } )
should be
exten => 2,3,Set(DB(forward/${CALLERIDNUM}) = ${FORWARD})
Note removal of the "$ {" and the "}"
