I would suggest the Pika Technologies Daytona cards. We have been beta-testing them with Asterisk for a while now and the results have been very, very good. The only drawback right now is that they only support 1.09. 1.2.x support should be ready later this quarter. Contact me off list if you would like more information.

In my expereince a channel bank with a digium single span card and a
Tellabs EC perfomed the best, but is too expensive (it gives you a
minimum of 8 ports). Next to that I use a mediatrix 1204, and compared
to all others I have tried works best. I have tried:
Sipura SPA3000
Digium TDM400 with 4 FXO mods
