If I repeatedly take a call on and off hold sometimes it will work,  
other times it they will hear distorted hold music, other times they  
will hear silence, the same thing happens with voice.

Driving me nuts :)

I have tried;

New firmware on the Polycom
New IOS on the router
2 x New Switches

Next is Asterisk version.
