In the phone config file add

proxy1_address: <provider 1 proxy>
proxy1_port: 5060
proxy1_register: 1

proxy2_address: <provider 1 proxy>
proxy2_port: 5060
proxy2_register: 1

Repeat these definitions for each provider then add the corresponding 
lineX_yyyy parameters as needed.
