Just a shot in the dark...I was getting this same thing with a TDM11B 
card.  Turns out I needed to add a pause before dialing on the FXO port; 
the first digit was getting lost randomly.  Adding a 'w' before the 
extension expression in the Dial command fixed it entirely.
