As I understand the present state of things the fact that VAD stops the
packet stream means that Asterisk loses its time base for the stream.
As others have mentioned already this is a pretty core thing. Not
easily changed.

Then again, that's the  beauty of open source. Roll up your sleeves and
dig into it...or if you're not a code jockey (I'm not myself) post a
bounty an try to stir up interest.

I would guess that VAD is not a big issue for many people or the entire
Asterisk community would not have come this far baed upon the existing
architecture.
