You shouldn't have outgoing in the same context.

Reorganise as follows:

[incoming]
include => extensions
include => mainmenu

[mainmenu]
exten => s...

[extensions]
exten => 100...

[outgoing]
ignorepat => 0 ...

[internal]
include => extensions
include => outgoing

Then you put context=internal for any phones that are internal (I.E. 
SIP/FXS/IAX etc), context=incoming for any FXO lines/trunks...
