Has anyone already implemented some form of intelligent call routing for
outbound calls from asterisk through FWD? 

FWD has a list of access codes to connect to specific providers, as the
providers are in different parts of the world, it is difficult to know
if the person you want to talk to is on a specific carrier or not.  Even
in some contries there are multiple providers with numbers scattered all
over.  

Since FWD (at this time) does not provide this routing on their end
(which would seem to be a better place that way it is updated as they
add/remove carriers) it must be done with asterisk.  

A macro in extensions.conf comes to mind, but rather than duplicate work
someone else has done I was looking for a mapping of numbers to given
carriers (not on a one by one basis, as that would be massive and
unmanageable).  Worst case I guess I could write something that would
try one after the other and if it fails move to the next access code, at
the end of the list either default to a paid service or error message.
Their list is small enough that isnt that hard to do, just not as
elegant as as their list grows it can add noticable delays in call set
up.
