Would this scenario work out better if the protocols were not single 
instance only, but could be loaded multiple times?

See the above request... Let's suppose that the SIP devices connecting 
internally were different than the ones connecting externally(they don't 
go back and forth). In this case, you could bind an instance of SIP to 
the external IP with all the configuration options you want there. You 
could also bind an instance of SIP internally, defining a whole 
different set of options(codecs, etc).

Just a thought. (I'm an idea person who wishes he was a coder.)
