You can get a Grandstream BT101 with this functionality already builtin -
configuration option 'Offhook Auto-Dial', same with the Handytone 286 for
use with an analog line.
Aside from these phones, I'm not sure in general how you would detect a
handset going off-hook - I don't believe there is any notify message in SIP
to notify an offhook event.
