This won't work with Firefly because qualify is broken in Firefly. It
doesn't respond to the IAX POKE messages and then Asterisk will
consider Firefly unreachable. Asterisk will not even try to dial out
to Firefly because the remote peer is know to be unreachable.

Put qualify=no and tell us again what the output of iax2 show peers is
after reload.
