I have been experiencing the same problem with my 4-port Digium FXS
card. I see Ouch, reset... messages on the console. One of the lines,
maybe 2, will either lose dial tone completely, or have tons of static
on the line. On the latest event, today, I noted that even when the
dialtone was gone, asterisk could still detect that I took the phone
off-hook.

To clear it up, either reboot, or, I find that bringing down asterisk,
then rmmod the wcfxs, and then modprobing it back, and ztcfg, and
restart asterisk, usually brings the lines back to life. Frequency:
maybe 1 or 2 times per week at the worst, once every other week at the
best.

I have the newest 4 port card. Yes, it is plugged into the power supply.
Yes, the TDM has IRQ #4, all to itself.

What is going on? Is there any hardware config that NEVER has any
problem?
