Or, use an spa-3000. I've got my spa set to 'always' dial out from the fxs
to the fxo for all calls, unless prefixed with an "8". All "8" calls (whether
extensions or sent to an itsp) are handled by *. The fxs is registering
with * as does the fxo (as a second extn). If power is lost, no change for
non-technical phone users and E911 is always supported.

Inbound itsp calls are sent to the spa fxs port using disttinctive ringing,
and all inbound pstn calls are spa pass-through (fxo -> fxs). Works great
and everyone gets every single function available with little spouse
training. :)
