I use a CAC channel bank.  Works perfect.  Lots of places you can pick 
up a patch panel, but if you want one with the amphenol connector on 
the back, I suggest searching ebay.  If you just want a regular patch 
panel with 110 connectors on the back, look at Fry's or Graybar 
(California at least).  Tons of places online sell them too.  Just 
google for them.  I personall like the Lucent panels, they seem to hold 
up well.
