It works because they have a separate SIP proxy that you log into, and
then the SIP proxy logs into their SIP server, all your calls pass
through that proxy.

You could do this yourself with a sip proxy like partysip.org, which
would, of course, require the server it is running on to have an
internet routable IP.  You could run it on the same server as asterisk,
just set it to use a different port, and configure your clients
appropriately.

This is why iconnecthere (and FWD) have separate config instructions for
NAT users.
