I'd argue that for practical purposes it is a limitation of the technology as it currently exists.
Typically you need support for these features in the phone itself in addition to the PBX/switch.
Most of the non-VOIP phones that support these features only interface with the particular PBX they
were designed for.  I've seen people mention that some of the higher end CISCO VOIP phones have some
support for intercom/paging, but I've not seen any posts about it actually working.
As far as I know, most currently available VOIP phones don't have the ability to support
intercom/paging, no matter what functionality you add to the asterisk.
