[asterisk-r2] mfcr2_metering_pulse_timeout no difference; outgoing calls being dropped

Thanks Moy. As always you're the man! 
This worked and our customer is happy. This seems to be related to
glitches that Telmex has on their lines and is loosing signaling for
very short periods of time 20 or 30 ms. We have some of their "planta
externa" out there now checking. We'll have to keep the pressure up to
get the source of this problem resolved. It looks like this is also
related to the bad copper quality and the Watson adapter they have
installed with the customer.

Here is the summary of what we have done to compensate for the state
changes originally mentioned.
We added the following line to /etc/asterisk/chan_dahdi.conf:
mfcr2_advanced_protocol_file=/usr/etc/r2proto.conf

In /usr/etc/r2proto.conf we commented out all lines except one:
timer.cas_persistance_check=100
