[asterisk-gui] Phone Provisioning: Set the ${SERVER} address?

Aha!  Thanks for finding that for me, Andrew.  For some reason, grep
didn't pick it up.  I must have used a bad string (I still can't find
it, hmm...).

Now, I just need to get the Linekeys field in the GUI to work.  It is
not used by polycom_line.xml.
