Sorry, I was on a work trip to California last week, and didn't have any
time time to get online.  (I was working like 18 hours a day, trying to
get a lot of Linux servers online in our data center.)  I just started a
new job, and so far I've been swamped with work and haven't been able to
do much on docs.  Hopefully that will change soon :-)

Yes, I've done quite a bit on the extensions.conf chapter, but it mostly
resembles what I posted to the mailing list a week or two ago. 
Hopefully I can work on it some more tonight and post an updated
version.
