Summary

Deadlock between dahdi_exception and dahdi_indicate when callwaiting is enabled on an TDM800P FXS port.
After initial deadlock was resolved, experienced deadlock with dahdi_hangup

Testing
-------

Using SVN-trunk-r272052M
Multiple calls from 2line SIP phone.
I also observed same deadlock under exactly the same test conditions with 1.6.0, 1.6.1 and 1.6.2
Didn't test asterisk 1.4
