Just switching to cdr_custom would do the job. Adapting cdr_csv to be as
adaptable as cdr_custom seems rather futile. Perhaps a standard
configuration for cdr_custom which exactly mimics the standard cdr_csv
setting, and then deprecate cdr_csv?
