You should be able to set this up and get it working.

Unfortunately, no.  Libpri does not support receiving path replacement messages to eliminate the tromboned call you describe.  I don't know how well the code will work for eliminating a tromboned call in the other direction will work.

I don't know if the H323 channel driver would eliminate tromboned calls.
