Greg,
 From the SiP debug output, it's clear that the ACK never reaches  
Asterisk. Asterisk retransmits 200 OK several times. Go from there and  
check.
Check the Contact in the 200 OK

Contact: <sip:2169203080 at 207.166.192.174>

And make sure that the Cisco can send to that IP.

But since the to: header is
To: <sip:2169203080 at 207.166.192.174>

  I guess it can.

Check that the Cisco receives the "Ringing" too and changes call state.
