[asterisk-dev] T.38 and call limits fixed in 1.4 - hopefully

There's been several issues with T.38 passthrough and - on a separate  
note - call
limits not being cleared during transfers.

Hopefully, I've been able to fix these issues with the help of Josh  
Colp, so please
test latest 1.4 from subversion so we can be sure of this before we  
release a new
version of 1.4.
