Hi!

I have similar problem when upgrade asterisk from 1.2.10 to 1.2.11.

setup: asterisk-1.2.11 + T100P + channel bank
description: one user(A) set Call Forwarding on her headset to forward
any calls to her cell-phone. another user(B) call to that extension,
then asterisk forward that call to A's cell-phone(through SIP), and
asterisk hangup immediately after A answer that call.
