which version of asterisk (1.2, 1.0.x or CVS HEAD) are you using ? i 
recently discovered a bug in CVS HEAD where the wrong cause code was being 
set for calls and thus causing a wrong 4xx/5xx SIP error response. my patch 
was deemed inaccurate (correctly, it seems) by kfleming and his patch fixes 
the problem cleanly and well. see http://bugs.digium.com/view.php?id=5394
