I am not sure why the autofallthrough parameter is relevant in this 
context anyway. Asterisk is not out of things to do when it hangs up the 
channel. It should go to the 't' extension. I think it is something new 
with the timeout code, such that the plan never arrives at the 't' 
extension.  That may be unmasking a pre-existing problem with the 
autofallthrough=no being ignored.
