Interesting but at this point in time don't you think it'd be most useful to
use ODBC like everything else in Asterisk?
I certainly don't mean to slam you since you've obviously put a large effort
into this, but what advantage does LCDial get me over a DBGet(${EXTEN}) or
even a simple small AGI that returns the first Dial() argument to use?
I have to say though that it is *very* refreshing to see documentation (and
very good documentation at that!) alongside the app instead of weeks/months
later (if ever) -- You deserve kudos for that, this is certain!

Well as I said most of asterisk is now using ODBC (c.f. the realtime stuff) so
I'd imagine that your first step would be to ODBCify it and make sure your
table creator conforms to standard SQL, then create a patch in the bugtracker
and let the bug marshalls have a look at it.
