Well, to be perfectly specific, MWI with Realtime _does_ work if you are 
using a "static" config from the database (where you just map sip.conf 
to a database table/view and it's read when you load/reload).

Where MWI with Realtime does not work if you use the "sipfriends" 
feature (the replacement for MYSQL_FRIENDS as you have already 
mentioned) to have SIP users/peers dynamically loaded as needed.
