[Asterisk-Dev] fxs woes

hello all,
ive been running asterisk for about 5-6 months now with a tdm04b and two tdm40b's. one fine evening all the fxs's decided to go bust. basically i dont get to hear the voice in some possible scenarios. the situation is:
sip->fxs fail
fxo->fxs fail
fxs->sip pass
fxs->fxo pass

the relevant log for fxo->fxs is as follows
