Hi.  In trying to compile our custom app against the latest asterisk, I'm
getting a:
/usr/include/asterisk/lock.h: In function `ast_mutex_init':
/usr/include/asterisk/lock.h:214: error: `PTHREAD_MUTEX_RECURSIVE'
undeclared (first use in this function)

I'm not sure where this is supposed to be getting #defined. <pthread.h> is
included in <asterisk/lock.h> so that's not it.

Any hints?  I've looked all through /usr/include and the asterisk source.
