Hi Joshua,

I did answer, but now I realize my reply was in "Draft" state, and I can't figure out how to actually publish it. So here is what I wrote:
"I wish I could, but it happened randomly (approximately once every two times). I did not find any reproducible pattern which would trigger the problem every time."
