Summary:                    ${VOICEMAIL_EXTEN} doesn't get set at asterisk
startup
Description: 
The ${VOICEMAIL_EXTEN} variable for phoneprov does not get set at startup. 
To get the variable set, one must manually reload from within asterisk
after startup.

Per the Asterisk maintenance timeline page at
http://www.asterisk.org/asterisk-versions maintenance (bug) support for the
1.6.0 and 1.6.1 branches has ended. For continued maintenance support
please move to the 1.6.2 branch.

Also, are you perhaps loading users.conf via static realtime?  This looks
like a load order problem.
