Summary:                    hangup on transfer
Description: 
when we use the asterisk feature:
atxfer => https://issues.asterisk.org/view.php?id=5                    ;
Attended transfer
we experience a hangup when we turn the "smart" transfer, into a "blind"
one. 
meaning, after a call is made between A and B, and A wants the transfer B
to C,
he press the right digits to play the DTMF's for "atxfer", get the ring
tone after the "transfer" playback, dial to extension C and press the pound
key.
when it starts to ring - the call between A and C (B is on hold) - A hangs
up to turn the "smart" transfer into "blind". the moment A hangs up, the
call is terminated for all sides.

the only error i found in the full log is -
"file.c: Failed to write frame"

This is typically called a "blonde transfer" as you shouldn't be using
attended transfers as blind transfers; that's what blind transfers are
for.

What does your dialplan look like so this can be reproduced? On the Local
channel, do you have a /n at the end?
