Summary: No hangup after attended transfer
Description:
A, B, C are all SIP telephones registered to OpenSER, which proxies all
calls to Asterisk. A calls B, which does an attended transfer to C. A and C
are then talking. C hangs up, but Asterisk never sends a SIP BYE to A.

It might be useful for you to remove OpenSER from the situation. I quickly
tested here with the above scenario and saw that the BYE was indeed sent to
phone A.
