Attached is a patch which allows multiple extensions to be created for
users in users.conf.  Has been in the Fedora Asterisk packages for a while
now.

We're using this the Fedora project's Asterisk server - we set up users in
users.conf using their username so that "sip:<username>@fedoraproject.org"
URLs work, but we also want numeric extensions to work as well for people
that have hard VoIP phones hooked up as well as for people using land lines
or cell phones to dial into our PSTN DIDs.
