Summary: hangupcause is always 0 if call made via call file
fails
Description:
Asterisk output gives this:
Channel 0/1, span 1 got hangup request, cause 28
So it is capturing the ISDN code but it is not being passed to the
hangupcause variable.

The reason for failure is available in the REASON dialplan variable, not
hangupcause. If you use that instead it should work fine.
