Summary: Asterisk crashes with error "Unable to masquerade"
Description:
A masquerade which fails with "Unable to answer" results in a second
masquerade. The second masquerade fails with "Unable to masquerade". A
moment later, channel.c says "Unable to find channel in list" and asterisk
crashes.

It's true that something caused the masquerade. The failure occurred on a
customer's PBX, so all I have to go by is the log files. From the log, I
would imagine that it was a transfer from one SIP phone to another. It is a
bit inconvenient that such events are not logged, even at DEBUG.
