Super-Phone.com can do this for you if u like.

Porting will cost you 15$ per DID number.

Out going calls are 1.9 cents a min and incoming are 15$ per month per channel, and 5$ per 
month per did.

You can provision pap2 or use sip account, however you like.

You can have the same on our hostedpbx on ip-pabx.com the price is the same accept you 
have 2$ per month per extention also, and you get call recording, and ivr for free.

You can signup for free for both the services to see how they perform for you.
