[asterisk-biz] U.S. 50 State Calling Prices for Non-Volume

Voicepulse connect has such a rate structure, but I never could get
consistent answers from them about the billing method for US48. Maybe
somebody here knows the answer based on actually using them. Is it 6/6
with fractional cents or what?
