If you intend to grow to over 8 lines, I would suggest looking at the T1
options.  You could use a T1 card and a channel bank, but if you also
are looking for true commercial grade Internet bandwidth, I would look
at the combined voice/data T1 offerings.

This is not possible to answer. PRI T1's (23 voice channels) can go from
$350ish to over $3000 per month.

I would suggest looking at a CLEC like Time Warner Telecom
(http://www.twtelecom.com).  They have an combined Voice/Data service
(VersiPak), that they do offer with a PRI output.

You could get 768K Data (CIR), 6 voice lines, and when voice lines are
not in use you can get another 384K of data.  I setup one consulting
customer (non-asterisk), with 768K CIR, and 12 voice lines on a PRI
output, bill is under $700/month.
